The Data Zoom deployment:
- A centralized data asset as a foundation for all of organizations reporting and analytics across multiple payers
- Build an ML based risk stratification model for chronic conditions to support population
Purpose:
- Ability to unify and integrate the claims data across multiple payer population
- Integrate Lab results data from industry leading lab vendors Quest, Labcorp
- Per population/ cohort sampling by various chronic conditions pertinent to the payer population
- Specifically predict cohorts for behavior health risk levels
- Using these cohorts and risk levels – define appropriate behavior health management programs
In addition, CHN kicked off a strategic program for RX Adherence to ensure patient compliance with following correct medication advice. The Data Zoom team collaborated with CHN to develop a RX Adherence capability
